Invoice Request Form

This form is to be used by employees and students requesting payments from Miftaah Institute for services or from event invoices. Receipts, invoices, and other relevant documentation must be uploaded below for the payments to be processed. Original receipts are required - All receipts must be itemized and show proof of payment. (Credit card receipts are not itemized, but they show proof of payment.)
